Some valves may have been over-tightened during the manufacturing process causing the cartridge to crack and the valve to fail.

Mountain Plumbing has received several reports of valves failing causing water damage to homes. No injuries have been reported.
Description
Mountain Plumbing has established a recall process of the MT6003-NL/XXX sold between April 11th 2011 and July 15th 2013. These valves have been previously recalled.
Remedy
If you believe you may have purchased and installed any of the valves listed, contact your plumber to have them replaced.
